Empatic is composed of two different drugs: Zonisamide and Bupropion, which are both on the market, though Empatic has not been approved yet, and so is unavailable.
Zonisamide has been used in the U.S. as an anti-seizure medication. Though it is unclear just how Zonisamide works, it is thought that it may possess mechanisms for “sodium-channel modulation and enhancement of dopamine and serotonin neurotransmission,” which latter mechanism could produce feelings of satiety. On its own, Zonisamide has also been shown to produce weight loss in clinical trials, and this may be attributed to its ability, in the hypothalamus, to release Monoamines, which stimulate the POMC neuron while suppressing the NPY/AgRP neuron. This dual function of stimulation and suppression may increase energy and reduce hunger.
Bupropion also works in the hypothalamus by stimulating the POMC neuron, which then releases a hormone that increases energy and reduces hunger.
The combination of Zonisamide and Bupropion works synergistically, according to Orexigen.com, in order to increase energy expenditure and reduce hunger “further than either compound alone.”

Orexigen.com is an extremely informative site, with animated illustrations of the science behind Empatic. One can learn about the neurons in the hypothalamus, the hormones they secrete, and the interactions of the various drugs Orexigen is testing. For those consumers used to the same old diet website, intent simply on selling a product, Orexigen.com might be a nice break. The site also maintains a thorough account of clinical trials performed.
